Participant: activity expense claim form

This form should be used by AYM participants/their parents/carers to claim expenses for taking part in AYM activities. This form is password protected and should only be completed when AYM have given approval that a claim can be made. Please use the password supplied by AYM to access the form.

Once completed, the claim will be checked and approved by a member of the AYM team.  Please allow 30 days for your claim to be processed.
